Membership Terms
Fees & Payments
- Monthly fee of $299, automatically billed to the card on file on the 1st of each month
- Membership fee is non-refundable
- Missed monthly treatments do not roll over unless specifically authorized
Term & Cancellation
- Minimum term of 12 months from the start date
- After the initial term, membership continues month-to-month until cancelled
- Early cancellation prior to completing the initial 12 months will result in a cancellation fee of $299 (one month’s fee)
- Cancellation requests must be submitted in writing at least 30 days prior to the next billing date
Exclusions & Limitations
- Membership discounts cannot be combined with other promotions or specials
- Membership benefits are non-transferable and may only be used by the member
- Medical services (such as visits, procedures, or treatments deemed medically necessary) are not included in this membership
